Accompanied documents required:

• Your Child's Birth certificate
• Citizenship Documents (if applicable)
• Proof of residence.  eg. a utility bill or rental agreement with your name and address.
• Copies of Family Court orders for confirmation of proof of name or custody arrangements (if applicable)
• An Australian Immunisation Register (AIR) Immunisation History Statement not more than two months old
